Tardy less than 50% of the scheduled shift

0.5 attendance point


Tardy more than 50% of the scheduled shift

 1.0 attendance point


Submitting this form is required but does not prevent attendance points from being assessed.


Attendance points are cumulative. Employees who exceed seven (7) points may be subject to termination.
